Editorial Commentary: This Is the Way: Extra-Articular Augmentation Is an Essential Consideration in Contemporary Anterior Cruciate Ligament Surgery

چکیده

Since the rediscovery of anterolateral ligament, extra-articular augmentation (EA) has evolved from controversial to an essential consideration in contemporary anterior cruciate ligament reconstruction surgery. Anterolateral (ALL) and lateral tenodesis are 2 common methods. Indications among early adopters pioneering at surgery included revision (ACL) case, chronic ACL tear, high-grade pivot shift, patients with hyperlax, hypermobile knees. Newer indications include young patient age, pivoting sport/high-demand/high-risk athlete, concurrent medial meniscus repair. Questions remain regarding best practices as continue evolve technique, graft choice, angle/position fixation, whether EA should be reconstructed routinely. This fast-moving surgical evolution serves a reminder key concepts; first, that tears occur more fundamentally setting rotatory instability, which soft tissue injuries common, and, second, even our “anatomic” reconstructions do not fully recapitulate native ACL, both give impetus reconstructing ALL.

Intra-articular Anterior Cruciate Ligament Reconstruction With Extra-articular Lateral Tenodesis of the Iliotibial Band

An increasing concern has been given to the rotation stability of the knee in the setting of an anterior cruciate ligament (ACL) reconstruction. This growing interest stems from a better understanding of the rotational stability of the knee afforded by the identification of the anterolateral ligament. Previously, a residual abnormal pivot-shift test had been found after an anatomic single-band ...

Anterior Cruciate Ligament Injury: A Consideration for Internal Brace Ligament Augmentation

As orthopedic surgery continues to evolve, it is indicated to reevaluate the surgical management of the anterior cruciate ligament (ACL) injury. Surgeons are searching for methods to shorten recovery time and to preserve mechanical neuro-sensory function. In doing so, they must choose whether to repair, or to reconstruct the ligament. Anatomical discoveries of the knee have led to the recent in...

Restoring Nature Through Individualized Anatomic Anterior Cruciate Ligament Reconstruction Surgery

Anterior cruciate ligament (ACL) reconstruction surgery has significantly evolved in recent years. This has led to development of new technologies that facilitate the diagnosis of ACL injury and the application of state of the art methods for treatment. In particular, individualized anatomical ACL reconstruction aims to restore native ACL function. Treatment is tailored to each patient based on...

Primary Anterior Cruciate Ligament Repair With Augmentation

Anterior cruciate ligament (ACL) tears are usually managed by reconstruction with autograft or allograft, but primary repair in carefully selected patients is a reasonable option. Proximal avulsions presenting early with excellent tissue quality are amenable to repair and healing. Restoration of native ACL preserves its proprioceptive and kinematic functions. A repair is less invasive and avoid...
